The Address Screen is for entering a Beneficiary's current mailing address, phone numbers, and email.

• Same as participant: checked by default, this checkbox indicates that instead of static address information, the beneficiary's address information should be set equal to the participant's.
• Mailing Address: use these fields to enter the current mailing address.
  
  The blue Lookup button can be used to find the two-letter state/province code.
• Phone: day and evening phone number are stored here.
• Email: for recording a beneficiary's email address.
