New Plan Wizard - Benefit Options

Benefit payment options available under the plan

Single Normal Form of Annuity: select your plan's single normal form of annuity. If you select a Certain and Life annuity as your single normal form, that option will automatically be checked in the options below.
Benefit Options Available: select all benefit options that are available to any participants within your plan. Note that a Life Annuity and 50% Joint & Survivor will be created automatically.
- 5 Years Certain and Life: check this box if a 5 Years Certain and Life annuity is an available option within your plan.
- 10 Years Certain and Life: check this box if a 10 Years Certain and Life annuity is an available option within your plan.
- 15 Years Certain and Life: check this box if a 15 Years Certain and Life annuity is an available option within your plan.
- 20 Years Certain and Life: check this box if a 20 Years Certain and Life annuity is an available option within your plan.
- 5 Years Certain Only: check this box if a 5 Years Certain Only annuity is an available option within your plan.
- 10 Years Certain Only: check this box if a 10 Years Certain Only annuity is an available option within your plan.
- 15 Years Certain Only: check this box if a 15 Years Certain Only annuity is an available option within your plan.
- 20 Years Certain Only: check this box if a 20 Years Certain Only annuity is an available option within your plan.
- 25% Joint & Survivor: check this box if a 25% Joint & Survivor annuity is an available option within your plan.
- 66 2/3% Joint & Survivor: check this box if a 66 2/3% Joint & Survivor annuity is an available option within your plan.
- 75% Joint & Survivor: check this box if a 75% Joint & Survivor annuity is an available option within your plan.
- 100% Joint & Survivor: check this box if a 100% Joint & Survivor annuity is an available option within your plan.
- 50% Joint & Last Survivor: check this box if a 50% Joint & Last Survivor annuity is an available option within your plan.
- 66 2/3% Joint & Last Survivor: check this box if a 66 2/3% Joint & Last Survivor annuity is an available option within your plan.
- 75% Joint & Last Survivor: check this box if a 75% Joint & Last Survivor annuity is an available option within your plan.
- 100% Joint & Last Survivor: check this box if a 100% Joint & Last Survivor annuity is an available option within your plan.
- PIA Level Income: check this box if a PIA Level Income annuity is an available option within your plan.
- Small Benefit Cashout: check this box if a Small Benefit Cashout is an available option within your plan.
- Lump Sum: check this box if a Lump Sum benefit (not including Small Benefit Cashout) is an available option within your plan.
Include Certain Periods with J&S Options: check this box if Joint & Survivor options should include a Certain Period. This option is only available if a Certain and Life has been selected as the single normal form of annuity. The certain period included in the Joint & Survivor options will match the certain period of the single normal form of annuity.
Develop factors in accordance with Rev Rul 76-47: select this option if you would like Conversion Factors to be developed in accordance with Rev Rul 76-47.
