Use this screen to select the benefit payments included in 1099-R reporting and form production.

• Period: enter the start and end dates for payments included in the report or forms.
• Plan Numbers: select one or more Plan Numbers, or check Include all plans.
• Benefit Types: select the benefit types to include, or check the option to include all benefit types.
• Payment Forms: select the payment forms to include, or check Include all payment forms.
• Benefit Payment Sets: select the Benefit Payment Sets to include, or check Include all definitions.
• Output: check Produce report, Produce 1099-R Forms, or both to choose the output generated when the screen is processed.
